We begin by examining the major arterial sources, including the single anterior spinal artery and the paired posterior spinal arteries, and describe how they course along the cord to supply its gray and white matter. We also discuss the contribution of segmental arteries, such as the radicular and medullary arteries, and highlight the clinical importance of the artery of Adamkiewicz in perfusing the lower thoracic and lumbar cord.

We then review venous drainage, covering the anterior and posterior spinal veins, their connections to the internal vertebral venous plexus, and the pathway of venous return. Special focus is given to the vulnerability of the spinal cord to ischemia, particularly in watershed zones, and the role of collateral circulation in preserving function.

Finally, we discuss anterior spinal artery syndrome, outlining its classic presentation with bilateral motor weakness, loss of pain and temperature sensation below the lesion, and preserved dorsal column function. We explain this vascular injury’s pathophysiology, common causes, and clinical implications.
