Join us in this lecture, where Professor Zach Murphy will discuss the anatomy and function of the accessory nerve, cranial nerve XI. We begin by detailing its unique origin from both the medulla oblongata and the spinal cord, highlighting how its spinal rootlets ascend through the foramen magnum before exiting the skull via the jugular foramen. The lecture examines the separation of the nerve into cranial and spinal components, clarifying their respective pathways and targets.

We focus on the spinal component’s motor innervation to the sternocleidomastoid and trapezius muscles, explaining how it contributes to head rotation, shoulder elevation, and maintaining posture. We also address the function of the cranial portion—though often integrated into vagus nerve pathways, noting its role in specific muscles of the larynx, pharynx, and soft palate.

Additional discussion explores the functional testing of cranial nerve XI during a neurological exam, including head-turning against resistance and shoulder shrug assessments. We connect these findings to the nerve’s clinical relevance in surgical neck dissections, trauma, and compression syndromes, emphasizing its importance in preserving functional neck and shoulder movement.
